Please ensure the authorization is HIPAA compliant and filled out in

Note: Most major hospitals, such as Kaiser, will not accept an authorization
unless it is HIPAA compliant.
Please provide as much information as possible such as the insured, the date of injury, and the facility's
claim number. If available, please also
include a copy of the ISO CLAIM INDEX.
Note: Some insurance companies, such as State Fund, will not accept
authorizations.
A Veterans Administration authorization must filled out by the claimant. For

here. Military records must be obtained through the National Personnel
Record Center and a separate VA authorization is required. For this authorization,

Note: Please specify whether the claimant was in the military or a
civilian working at a military base.

In order to obtain medical records from Kaiser, we must issue separate subpoenas or authorizations for Kaiser Hospital and SCPMG (Southern California Permanente Medical Group). Kaiser requires separate subpoenas as the hospital holds inpatient records and SCPMG holds outpatient records.
Note: Unless otherwise specified on your order, two subpoenas will be generated for each Kaiser facility
Kaiser maintains their records in separate regions within larger counties. The possibility exists that patient records overlap two separate regions. In order to obtain all records of the claimant, we are often required to serve paperwork to two or more separate Kaiser facilities. With this in mind, it is important for us to know the locations where the patient was treated.
